• public library
• post office
• community center
• recreation center
• or other public, civic asset

Design Certification

Submit all of the following:

1. Annotated map(s) or plan(s) showing
   1. If it will be located on-site, the location of the qualifying civic resource within the project boundary
   2. If located off-site, the distance between the qualifying civic resource and the project boundary
2. An official or draft statement confirming the qualifying civic resource that will be included.
3. If existing, a minimum of 3 annotated photographs of the qualifying civic resource.

Built Certification

Submit all of the following:

1. Annotated map(s) or plan(s) showing
   1. If located on-site, the location of the qualifying civic resource within the project boundary
   2. If located off-site, the distance between the qualifying civic resource and the project boundary
2. An official statement confirming the qualifying civic resource that is included.
