   2. If located off-site, the distance between the qualifying facility and the project boundary
2. An official or draft statement confirming the type of qualifying healthcare facility that will be included.
3. If existing, a minimum of 3 annotated photographs of the qualifying healthcare facility.

Built Certification

Submit all of the following:
